Need Help with Access?

We are using 2 systems to serve you. One is where your medical records, forms, programs and classes are, and the other is our online store, where our patents can purchases their prescriptions.

Pharmacy correspondence & Prescriptions

Please allow at least one week for processing.

Do not have your pharmacy fax us, this is the most difficult form of communication available now.

Do not text Sarah, she has no way to track texts and they are abundant and endless and you will get lost.

You will need an appointment for most refills. State law determines most of this but we try and prescribe enough to get you to your next check in or required visit so it is likely best to just make an appointment.

Our Locations & Hours

We have moved! We are now in the office on Tuesdays and Thursdays from 3-6 in person, please call or text to verify we are in that week.

If you need to ask your provider a question, the best way is to make an appointment.

Our providers simply can not keep up with any question, refill or anything that requires their time without an appointment. This includes paperwork of any kind. Anything that requires time, of any length. You can make a quick visit most days, schedule online. For emergencies, head to the ER or dial 911.
